A housekeeping service provider evaluation should be part of an annual review at a minimum. This form will help you make an informed decision about whether or not to renew your contract. You can then meet with your provider and discuss areas for improvement in the coming year. You can then share a list of approved providers for the year with your purchasing staff.

You will have two documents to download for this purchase: The evaluation form and the list of approved suppliers.
